20% GRATUITY IS AUTOMATICALLY INCLUDED ON ALL TABS AND FINAL BILL.

ICHIBANG TAB is to be closed on ICHIBANG Side. BAR TAB is to be closed on BAR Side. If you open on both side, please be aware that they are SEPARATE and to be closed on both sides.

I understand our party is responsible for keeping track of the time. Time in the room DOES NOT STOP until the all the microphones (and/or) iPad have been brought back to the front desk to check out.
Customer Aggreements
In consideration of your participation in the foregoing, the undersigned acknowledges and agrees to the following:

NO OUTSIDE FOOD, DRINKS, ALCOHOL or SPIRITS per Texas TABC Alcohol Code 28.10. *Violators will have their bottled be confiscated and FINED $200 PER BOTTLE. NO EXCEPTIONS.

ZERO TOLERANCE FOR UNDERAGE DRINKING or ANYONE found ordering for a minor will be asked to leave. *Refer to www.dps.texas.gov.

NO VAPING OR SMOKING ON PREMISES. NO EXCEPTIONS. *Violators will be fined $200

ZERO TOLERANCE FOR OR ANY ASSOCIATION with DRUG USE, GAMBLING, or ANY ILLEGAL ACTIVITY in or around the premises, including the parking lot. Suspected violators will be promptly REPORTED to the Authorities and asked to vacate the venue immediately.

NO CONFETTI, GLITTER, FEATHER or decorations that could damage the walls or other surfaces. Violators will be FINED $200.

Any damage to the SOUND EQUIPMENTS, will result in automatic FINE OF $400.

Any damage to the TV, will result in automatic FINE OF $700.

Any damage to the iPad [selected rooms], will result in automatic FINE OF $500.

Cleaning fee of minimum $100 will be added to the room on the final bill for excessive dirtiness or bodily fluids.

As the person signing this form, It is MY responsibility to notify ALL guests of the house rules, and any cost involved. I understand that any items ordered by my guest, but is not paid, will be added to the room fees that I’m responsible for.
